First your group needs to download and complete a contract with LAYC to allow us to process PVG Scheme applications on your behalf.

Then the individual and children's worker should complete the applications on your behalf filling in the form below using BLOCK CAPITAL letters (we suggest pressing the CAPS LOCK key on your keyboard).

The information the worker provides is manually entered onto a PVG Scheme Application which they will be required to check and sign at the identification verification stage of the process, for which we will require at least three forms of identity. If possible one of them should be photographic. These should include two recent proofs of address, such as a utility bill and a bank statement.
